Clear segmented worms?

I just took a look at my 20 gallon....and had to do a double take.

There are, as of now, at least TEN of these strange worm-looking things in the gravel, with one end waving in the water. They range from an inch to 1.5 inches, are segmented, and white-clear.

What the heck ARE they????

I first thought they were bits of hornwort that fell from the main plant, but these guys are moving by themselves....

What are they and how do I get rid of them?

Tank: 20 gallons
AC 200 Filter
Java moss and hornwort
2 female bristlenose plecos
3 albino cories
2 mollies

The only thing that I've been able to figure out is they may be tubifex or some similar species. I haven't seen them in my tanks, but I have a friend with them. Do a google image search for tubifex and see if they are the same. As far as you can away from your plants do a little gravel sphyon. Put the water in a white wide basin or ziploc bag on top of white paper. See how many you get. It may be easier to get a good look at them like that. Plus you might get to see what else is in there. My friend had at least 3 different visible creature in there.
