Dying swordtail

I bought three swordtails a couple months ago, one male and two females. One of the females died about a month or more ago for some unknown reason and I didn't think much of it. Now my male is missing and probably dead, and my last female is on the floor of the aquarium, her fins are tearing, shes not eating, and she seems to have with stuff on the top of her head and at the edge of her fins. Is this a disease or something that can be treated? Some things I think might of caused it is stiring up the gravel and a lot of the dirt on the bottom got disturbed or I've been using topfin pH increase and Rid Ich+.

37 gallon
3 angels, bushynose pleco, 8cardinals, 6 black neon tetras, 3 black tetras, 5 kuhli loaches, and the swordtail

pH is 6.4
Alkalinity is 0-80 ppm
Hardness is about 120 ppm
Nitrate is between 40 and 80ppm
Nitrite is turning some light grey tanish color not on the test so I'm assuming its between zero and 0.5ppms.
I changed the water a week ago. pH increase and Rid Ich+is the only chemical I've used but angel is the only one with ich. a lot of my plants also havn't been doing much growing and are kind've just sitting there if thats somehow related.

Sorry we need more info. What are all your water test results? Tank size & stock?
What chemicals have you used and when diod you last vaccuum the gravel?
